Assessing the Apical Pulse

Equipment
■ Watch with a second hand or digital readout.
■ Stethoscope.
■ Alcohol wipes.

Assessment
■ Determine why assessment of the apical pulse is indicated.
■ Assess factors that may alter the pulse, such as activity and medications.
■ Be Smart! If the client has been recently active, wait 10 to 15 minutes before obtaining measurement.

Key Points
■ Position client sitting, if possible, or supine if not.
■ Palpate and place stethoscope at the 5th intercostal space in the midclavicular line (PMI, the apex).
■ Count for 60 seconds.
■ Note pulse rate, rhythm, and quality, and the S1 and S2 heart sounds.

Documentation
■ Note and document:
■ Pulse rate, rhythm, and site.
■ Data and other factors supporting your findings (e.g., skin color).
■ Trends over time.
■ Be Safe! Note whether findings are within normal limits. If not, note other factors supporting the findings; notify the primary care provider.
